Enrollment Packet
The Enrollment Packet consists of: Student Checklist, Registration Forms, Health Forms, Parent Handbook, and Immunization Card. An updated Immunization Card is also required. Both the Health Evaluation Form and the Immunization Card must be updated on an annual basis. The Enrollment Packet must be completed before care begins for your child.
